Personal Information Collection Notice

1.Introduction

This Personal Information Collection Notice (Notice) is provided to you by Azhar Pty Limited (ABN 33 624 443 947) (we, our and us) to notify you of certain matters relating to how we collect, handle and disclose your Personal Information as required under the Privacy Act 1988 (Cth) (Privacy Act).

When we refer to "Personal Information", we mean information or an opinion about an identified individual, or an individual who is reasonably identifiable, whether the information or opinion is true or not, and whether recorded in a material form or not. It includes sensitive information, such as the developmental and health information you enter about your child.

Where you have been provided this Notice by us, we have done so to notify you that we have collected, or will be collecting, your Personal Information.

3.How we collect Personal Information

We collect your Personal Information directly from you, the account holder, through the App, when you create an account, create a child profile, use the App’s features, post in the community, or contact us for support.

4.Purpose of collection

We are collecting your Personal Information for the following purposes:

   to provide and personalise the App and its features to you;
2. (b)
   to securely store your child’s records, logs and media;
3. (c)
   to manage your account, your subscription and any free trial;
4. (d)
   to deliver the community and the questions-to-our-OT-team features, and to keep the community safe; and
5. (e)
   to provide support.

Your child’s developmental and health information is sensitive information, which we collect only with your consent.

5.Consequences if Personal Information is not collected

Without your Personal Information, we may not be able to provide the App’s features to you, including creating a child profile or answering questions you submit to our occupational therapy team.

6.Disclosure of Personal Information

We usually disclose Personal Information of the kind we collect from you to our service providers who help us run the App, such as our hosting and storage, subscription-management and notification providers. We do not sell your Personal Information.

Some of these providers are located overseas, commonly in the United States.

7.Our Privacy Policy

Our Privacy Policy, available in the App, contains details about how we handle Personal Information, how you may complain about a breach of the APPs and how we deal with such a complaint, and how you can access your Personal Information and seek its correction.
